Travel Soccer Registration Documents:
The following items MUST be submitted to your team manager as a package before your child can participate in league games! Please do not submit them piecemeal.
- One copy of child’s birth certificate
- One notarized copy of the medical release form (above)
- Two passport sized photos, approximately 1.5 inches by 1.5 inches
- Signed Code of Conduct form (below)
